Cyclic Neutropenia

Cyclic neutropenia is an uncommon congenital blood disorder distinguish by abnormally low levels of certain white blood corpuscles (neutrophils) in the blood. It presents itself with frequent infections in the body like repetitive episodes of fever and mouth ulcers.

Incidence

It occurs in one person in one million people in the world.

Cyclic Neutropenia is caused because of autosomal dominant innate mutations in ELA2 gene. This is the gene which encodes the gene neutrophils elastase. The gene is released when the body effort to fight in opposition to any infection in the body. But because of their absence, the bone marrow needs time to replenish the neutrophils supply. This neutrophils deficiency tends to occur every 3 weeks and lasts for 3 to 6 days.

Symptoms

The episodes of neutropenia become noticeable after birth. The cycle of neutropenia frequently comes after every 21 days lasting for 3 to 4 days.

  • Oral ulcers
  • Swelling and inflammation of gums (Gingivitis)
  • Recurrent fever
  • Generalized weakness and malaise

Investigations

Patients with cyclic neutropenia should be genetically tested for ELANE gene.

Blood investigations like complete blood count, total leukocyte count and differential leukocyte count should be performed. The neutrophil count should be monitored twice in a month.
